NATIVE HEALTH Central is located at the southeast corner of Central and Indian School in Phoenix. NATIVE HEALTH Central provides primary medical, dental, behavioral health, and community health and wellness services to urban Native Americans and other community members residing in the metro Phoenix area.

NATIVE HEALTH Central is located near the southeast corner of Indian School Road and Central Avenue. We are conveniently located on the light rail line (Indian School/Central stop) and at the corner of two bus lines (Central Avenue and Indian School). Our location provides additional dental operatories and medical exam rooms within a 27,000 square foot building.

In efforts to provide high quality care to our patients, NHW Community Health Center maintains accreditation through Accreditation Association for Ambulatory Health Care, Inc.

The Accreditation Association for Ambulatory Health Care, also known as AAAHC, is a private, non-profit organization formed in 1979. It is the preeminent leader in developing standards to advance and promote patient safety, quality and value for ambulatory health care through peer-based accreditation processes, education and research.

Accreditation is awarded to organizations that are found to be in compliance with AAAHC standards. AAAHC currently accredits over 5,000 organizations in a wide variety of ambulatory health care settings, including ambulatory and office based surgery centers, managed care organizations, as well as Indian and student health centers, among others. With a single focus on the ambulatory care community, AAAHC offers organizations a high quality and collaborative approach to accreditation.

  • Monday-Friday, 8:00 a.m.-5:00 p.m. Emergency walk-in appointments available between 8:00 a.m.-3:00 p.m., first come, first served.

Behavioral Health Services:

Monday-Friday, 8:00 a.m. -6:00 p.m. 

Behavioral Health Intakes are by appointment and a walk-in basis. Bring required documents and copies of court orders. Please arrive at least 30 minutes early for your intake appointment to complete necessary paperwork. All AHCCCS plans are accepted. NATIVE HEALTH is an AHCCCS Complete Care provider. Community Health and Wellness Services:

  • Health Start – New mother’s program for pregnant women or with children up to age two. For more information call (602) 279-5262 x 18010.

Additional Services:

  • Arizona Department of Economic Security assistance with AHCCCS, TANF, and SNAP. Monday-Friday, 8:00 a.m.-4:30 p.m. First come, first served. Please bring photo ID and any necessary documents. 
  • Arizona Department of Economic Security Arizona@ Work assists people with finding employment, resumes, interviews, and more. Tuesdays and Wednesdays, 8:00 a.m.-4:30 p.m.
  • Arizona Department of Economic Security Veteran’s Services Toolkit – Thursdays, 9:00 a.m.-4:00 p.m.

As a Federally Qualified Health Center, NHW Community Health Center is eligible for Federal Torts Claim Act (FTCA) medical malpractice liability protection. As such, NHW Community Health Center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
